Registration

Happy 2010

It is time to register for Spring/Summer 2010 soccer:

All registration forms are accessible under FORMS.   For questions related to the registration process please do not hesitate to e-mail  Mark Hepp at teresehepp@comcast.net or call him at 763-535-8466.

Recreational Soccer Player registration Information:

Please submit your registration and payment to the address shown on your registration form by March 31st, 2010.  This includes completed registration form, front and back, including parent signature and full payment.  For specific questions on the recreational program please contact Vice President of Recreational Donna Ostdiek by phone at 763-545-1122 or by e-mail at jdostdiek@comcast.net.

Competitive (Traveling) Player registration Information:

  • Returning players who have been assigned to a competitive GV Phoenix team - please submit your registration and final payment to your coach or manager as soon as possible - they will need to turn all team registration materials in by Friday, January 15th, 2010.  This includes completed registration form, front and back, including parent signature; and full payment, including uniform payment if necessary.
  • New players who have been assigned to a competitive GV Phoenix team - please submit your registration and final payment to your coach or manager as soon as possible - they will need to turn all team registration materials in by Friday, January 15th, 2010.  This includes completed registration form, front and back, including parent signature; full payment, including uniform payment if necessary; and a photo copy of player’s Birth Certificate or passport for all new competitive players.  This is mandatory by the MYSA.  NO BIRTH CERTIFICATE OR PASSPORT, NO PASS!  NO EXCEPTIONS.

  • New players who have  not assigned to a competitive GV Phoenix team - please submit your registration and payment to the address shown on your registration form by Sunday, January 31st, 2010.  This includes completed registration form, front and back, including parent signature; full payment, including uniform payment if necessary; and a photo copy of your player’s Birth Certificate or passport.  This is mandatory by the MYSA.  NO BIRTH CERTIFICATE OR PASSPORT, NO PASS!  NO EXCEPTIONS.

Competitive (Traveling) Team Information for Managers and Coaches:

Teams staying together should have their team manager or coach collect all registration materials and submit them directly to the Club Registrars.  Please e-mail Mark Hepp  at teresehepp@comcast.net or call him at 763-535-8466 to let them know when you are planning to drop off registrations.  Their address is:  8525 Fairview Ave N., New Hope, MN.

Please keep a photo copy of all your team registration forms and make sure your registration package includes all of the following: 

  • Team age and grouping (U9 boys, U10 girls, etc….).

  • Level your team will play for fall:  Maroon or Gold for U9 & U10; C2 or C3 for U11 teams; C1, C2, or C3 for older teams. The MYSA rules are U9 to U13 teams can "self select" the level they want to play.  After U13, it's determined by their standings in the previous summer season by the MYSA next level (i.e.: C3 team with 88% wins is promoted to C2 the following season).

  • Completed registration forms, front and back, including parent signature.

  • Full payment, including uniform payment if necessary, for each player.

  • A photo copy of player’s Birth Certificate or passport for all new competitive players.  This is mandatory by the MYSA.  NO BIRTH CERTIFICATE OR PASSPORT, NO PASS!  NO EXCEPTIONS.

  • Team name.  This is the name that will print on the MYSA standings and on the players’ passes and official rosters.   Your last team name will be used or if you are a new team, a team name will be assigned if a new name is not indicated.

Coaches and Managers complete the on-line background check.  Prior to registering a team all coaches and managers must complete the on-line background check

If you had a team in the fall this background check should be complete – check with Mark if you are unsure.  For all others this is the start of the new soccer year and it is required that all club volunteers/adults enter their background check information on-line at this website:  http://www.mcdowellagency.com/

The Golden Valley Phoenix Soccer Club Golden Valley Phoenix Soccer Club volunteer/adult login username & password are the same:  MYSGVL.  This is a secure website.  If you have any specific questions concerning security please contact the McDowell Agency directly.  No volunteer/adult will be activated with the club or added to a team until I have received a background check completed status confirmation report.

Scheduling:  All games will be played on Mondays/Wednesdays or Tuesday/Thursdays depending on the specific game night for your age and level.  All teams will have the opportunity to submit game conflicts in mid March prior to preliminary schedule being created.  The MYSA may be able to schedule around be able to schedule around your conflicts.  The MYSA mandatory coaches meeting will take place in April.  More information will be sent out when date and time are known.  A coach and/or manager from every team must attend this meeting.

Adding Players:  Players may be added after the season starts if you have openings on your roster.   You must allow a possible 7 day turn around for a players pass.  Each Club is only allowed to submit once per week to the MYSA for passes.  For emergencies, a fine of $25.00 must be paid upon picking up the pass(es).  Replacement passes are $8.00 per pass.  Coaches or Team Managers may need to pick up their own passes after the initial mailing of the passes to the Club Registrar prior to the start of the season.  (Note:  these amounts may have increased.)
